Physical exam for relevant underlying medical conditions and functional status

Underlying diseases, or underlying medical conditions, are frequently not presented in the history taking. Therefore, we do some routine medical exams. However, you should correlate with the complaints.

Dyspnea/chest discomfort - structural heart diseases, lung diseases, cancer, kidney diseases
Dyspepsia - hepatobiliary diseases, DM, previous surgery/trauma
Jaundice - hepatobiliary diseases
Dizziness - same as dyspnea plus DM
Edema - structural heart diseases, cancer, kidney diseases, previous surgery/trauma
Confusion/proximal muscle weakness - neurological deficit/diseases, kidney diseases, any endocrine diseases, cancer

A stepwise approach is physical examination from head to toe, starting at vital signs, general examination, head, neck, chest, abdomen, back, perineum, lower extremities, and upper extremities; and then; further examinations by systems; neurologic examination.

You will have to focus on examination for chief complaints and relevant underlying conditions and revelant physiology/pathophysiology, or you will find only scattering findings which you cannot interpret, and forgetting some important or helpful examinations

Incidentaloma and incidental findings

สืบเนื่องจาก post ก่อนหน้านี้ มีการพูดถึงว่า การตรวจร่างกายและ lab โดยไม่ดูคนไข้ จะนำมาสู่ overdiagnosis และการรักษาไม่ตรงกับโรค

จริงๆผมว่า chief complaint สำคัญที่สุด และการ diagnosis จะต้องนำมาสู่การรักษา chief complaint แบบ holistic

การซักประวัติโดยไม่ focus ไม่ดู chief complaint ก็น่าจะมาสู่การพบ incidental finding ได้ รวมทั้งการตรวจร่างกายย่อมพบ incidental finding ได้เช่นเดียวกัน ที่พบบ่อยเช่น heart murmur หรือ abdominal aortic aneurysm

การพบ incidental finding โดยการซักประวัติและตรวจร่างกายย่อมเป็นสิ่งที่มองเห็นเป็นของธรรมดาและไม่น่าเป็นเรื่องผิดอะไร แต่ incidental finding ที่มักได้รับการกล่าวถึงว่าไม่ควรตรวจเลย และนำมาสู่ค่าใช้จ่ายที่เพิ่มขึ้น (increased cost) และความเสี่ยงที่เพิ่มขึ้นจาก invasive procedure ที่ใช้ในการสืบค้นเพิ่มเติม ก็คือ lab and imaging

ส่วนตัวมองว่า overinvestigation and overdiagnosis ไม่ใช่เรื่องผิด ถ้าไม่ได้เพิ่ม risk ให้แก่คนไข้ แต่ว่าความเป็นจริงแล้ว เรามีโอกาสเพิ่ม risk ให้คนไข้ได้มากมาย โดย

  1. ทำให้คนไข้ได้รู้ในสิ่งที่ยังไม่จำเป็นต้องรู้ เช่น ตรวจพบ tumor ใน CT โดยที่ผู้ป่วยยังไม่ได้เตรียมใจมาก่อน และไม่สัมพันธ์กับ chief complaint (แต่ว่าถ้าตรวจพบโดยการตรวจร่างกายมักไม่มีใครว่านะ)
  2. มี investigation เพิ่มเติมโดยที่แพทย์ไม่ทราบว่า benign
  3. ระหว่างที่คนไข้รอฟังผลตรวจเพิ่มเติม ก็เป็นการเพิ่มความกังวลให้กับคนไข้ไปอีก
แล้วแต่จะมองนะ แต่ท้ายสุดแล้ว แนวทางแก้ปัญหา คือ แพทย์ต้องรู้ก่อนว่าจะพบ incidental finding อะไรได้บ้าง และ advise ผู้ป่วยก่อนการตรวจเพิ่มเติมเสมอว่า 1.ต้องการตรวจเพิ่มเติมไหมและจะได้ประโยชน์อะไร 2. ถ้าตรวจพบ incidental finding ต้องการทราบไหม หากแพทย์คิดว่าไม่เป็นอันตรายต่อผู้ป่วย
