Ventilator setting in sepsis with ARDS or severe metabolic acidosis

ดีที่สุด คือ ใช้ protective lung strategy
TV: 6-8 ml/kg
Plateau pressure <30
RR <30
I:E 1:1 หรือ inverse ก็ได้ถ้าจำเป็น
เพิ่ม PEEP ได้
Minute ventilation ไม่ต้องสูงมาก 7-8 ml/kg/min ดีที่สุด
(Hyperventilation --> controversial)
Permissive hypercapnia ก็ได้ keep pH >7.2
ไม่ต้องสน PaCO2
O2sat 95-99% (อาจจะต่ำกว่านี้ได้ ไม่แน่ใจ)
Sedate ได้

ถ้า pH ต่ำมาก ให้ NaHCO3 ได้ keep pH >7.0-7.2
Dose NaHCO3 ต้องมากพอ คิดตาม bicarbonate space

อาจจะมากถึง 3-5 amp ต่อชั่วโมงหรือมากกว่านั้น
Repeat pH อย่างช้าใน 3-4 hr
แนะนำให้ drip ไม่ push ยกเว้นใน CPR

7.5% NaHCO3 1 amp มี 50 ml และมี 0.89 mEq/ml

สุดท้ายคือ ต้อง treat cause

Rapid treatment of shock to prevent complications

คำว่า shock คือ เลือดไปเลื้ยง tissue ปลายทางได้ไม่เพียงพอ เมื่อมองจากหัวใจเป็นศูนย์กลางจะพบว่ามี 3 components เป็นอย่างน้อย

  1. Preload (blood volume)
  2. Contractility and heart rate
  3. Afterload (arterial vascular tone)
ไม่ว่าจะเป็น shock จากสาเหตุใดจะมีหนึ่งในสาม components เหล่านี้ด้วยเสมอ

แนวทางการแก้ไข shock อย่างรวดเร็ว ผมยึด guideline จาก septic shock ก็คือ EGDT ในกรณีของ septic shock จะมี afterload เสียไป และอาจมี contractility เสียด้วย

เป้าหมายในการ resuscitate ที่ต้องทำได้อย่างรวดเร็วคือ เพิ่ม tissue perfusion สิ่งที่ใช้ monitor ได้ คือ serum lactate level และ ScvO2 เป้าหมายคือ serum lactate <3 และ ScvO2 >70%

ก่อนที่ serum lactate และ ScvO2 จะ normal ได้นั้น ต้องมี preload และ ฺBP ที่เพียงพอก่อน preload ประเมินได้ด้วย central line วัด CVP ต่อให้ใส่ Swan-Ganz catheter ได้ ถึงจะได้ตัวเลขที่แม่นยำกว่า แต่เขาก็ไม่บอกว่า prognosis จะดีขึ้นหรอกนะ

ส่วนเรื่อง BP นี่เป็นตัวใช้ diagnosis sepsis เมื่อ MAP <70 และต้องแก้ไขให้ MAP >65

ต้องเร็วเท่าไร เขาบอกว่า ภายใน 6 ชั่วโมง แต่ผมขอบอกว่า volume กับ BP เป็นสิ่งที่ต้องรีบแก้ไขให้เร็วที่สุด เอาเป็นว่า ภายใน 1-2 ชั่วโมง อย่าปล่อยให้ shock นาน ถ้ามันนานมาก ก็รีบ consult พี่ dent หรือขอ ICU ซะ

แก้ไข BP ให้ใช้ vasopressor เช่น norepinephrine โดยไม่จำเป็นต้องรอให้ volume เพียงก็ได้ ถ้าคิดว่า volume จะยังไม่เพียงพอใน 1-2 ชั่วโมง อย่าปล่อยให้ BP drop นาน

vasopressor เช่น norepinephrine ก็มี high dose นะ norepinephrine(4:100) >15 ml/hr, (4:250) >40 ml/hr ก็คือ >20 mcg/min ถือว่าเยอะไปแล้วนะ โดย rule of thumb ผมถือว่าอะไรที่ vasopressor หรือ inotrope rate >20 ml/hr ถือว่าเยอะเกินไป

ถ้า BP drop นาน จะมี DIC และ AKI ตามมาได้

เรื่อง goal เรื่อง lactate นี่ เราสามารถดูจาก blood gas ได้นะ ก็คือ ตัว base excess นอกจากนี้อาจดูได้จาก anion gap, HCO3, pH

Definition of shock and its impact on outcome (my ideas)

A usual way to define shock is MAP <= 65 or SBP <= 70, but why? What about the other parameters?

It is said that when the MAP requires to keep adequate perfusion to brain and heart is MAP >=70. I have seen the number >= 60, too. The MAP should probably be from invasive intra-arterial BP. We are talking about brain and heart, so a good clinical sign to monitor is consciousness. This would correlate with cerebral perfusion pressure (CPP) and cerebral blood flow (CBF).

What about the other end-organ damages as a result of shock? The most important one would probably be the kidney. To prevent "acute kidney injury," we need adequate "renal blood flow." To get enough blood flow, we need both adequate cardiac output and allocation of blood to the kidneys. When there is inadequate blood pressure to brain and heart, peripheral vasoconstriction occurs, and it does not spare the kidneys, resulting in inadequate renal blood flow. To reallocate blood to the kidneys, peripheral vasoconstriction needs to resolve, by giving adequate fluid volume. People sometimes talk about vasodilators too, but I won't be talking about that now.

Other end-organs, such as GI tract are less important, so we are not talking about that now.

SBP is another parameter. There is a goal in septic shock saying we should keep MAP >= 90, but I am not sure about that.

DBP is a less talked-about in shock. However, SBP minus by DBP, or the pulse pressure, correlates with stroke volume.

However, I have heard that in acute hypertension, keeping DBP < 110, does prevent end-organ damage.

Pulse, or heart rate, is another thing to monitor. It does tell you about decrease in blood pressure to carotid sinuses and aortic arch, and the resultant reflex tachycardia, therefore implying the physiology.

Some other interesting parameters are, CVP, PCWP, and cardiac index. I will probably be talking about that later.
