Malaise with fever -- may be influenza-like illness. Common provisional diagnosis - common cold.
Malaise without fever -- may be a prodromal symptom for anicteric acute viral hepatitis, so don't forget to consider probability. May also be just depression or something else.
If psychosomatic, may be associated with GERD, functional dyspepsia, IBS.
